Job Description

About the Role:

As a Care Partner at SwanCare, you will deliver care management activities that enable older people to live safely and independently in their own homes.  You will collaborate with participants, their supporters and interdisciplinary teams to coordinate tailored, responsive and inclusive support.  Relationship management, care planning and coordination are key elements of this role.   Available on a part-time basis, 15 hours per week with the flexibility to increase hours when and if required.

Key Responsibilities:

  • Collaborate with participants to develop individualised care plans.
  • Build meaningful relationships with participants and their supporters to understand goals, preferences and life circumstances.
  • Collaborate with clinically qualified Care Partners, escalating clinical decisions where participant’s needs exceed the general care management scope.
  • Application of Pathway-Specific Coordination including Restorative Care Pathway, End of Life Pathway and AT-HM Pathway.
  • Regularly monitor and review individualised care plans and escalate when required.

Desired Skills and Experience

Key Role Requirements:

  • Diploma level qualification in Community Services, Aged Care, Case Management or equivalent experience.
  • Demonstrated experience in care coordination, case management or similar role in aged care, health or disability services.
  • Competent in the understanding of the Support at Home Program, Quality Standards and aged care rights.
  • Demonstrated ability in the interpretation of clinical guidance.
  • Strong communication, empathy and cultural competence skills.
  • Technically savvy! 
  • Current C Class Drivers Licence.
  • National Police Clearance.
  • Permanent working rights in Australia.
