Job Description

About the Role:

Reporting to the Home Care Manager, the Clinical Care Partner plays a key role in supporting older people to live safely and independently at home. Utilising a rights-based, person-centred approach, you will deliver high-quality care management and coordinate tailored, responsive and inclusive support across care pathways.

In this relationship-focussed role, you will work closely with participants, their supports and interdisciplinary teams to plan, monitor and coordinate services. Overall, promoting wellness, reablement and culturally safe care. 

Key responsibilities:

  • Develop and review individualised care plans aligned with assessments, goals and clinical advise.
  • Build strong relationships with participants and supports to understand needs and preferences.
  • Coordinate services across Restorative Care, End-of-Life and Assistive Technology/Home Modification pathways.
  • Monitor clinical interventions, identify emerging risks and escalate appropriately.
  • Manage participants budgets, monitor spending and ensure compliance with Support at Home requirements.
  • Provide education on aged care navigation, AT use, wellness and health literacy.
  • Maintain accurate documentation and collaborate effectively with internal and external stakeholders.

Desired Skills and Experience

Key Role Requirements:

  • AHPRA registration.
  • National Police Clearance.
  • Strong understanding of the Support at Home Program, Quality Standards and aged care rights.
  • Ability to interpret clinical guidance and manage risk.
  • Excellent communication, empathy and cultural competence.
  • Confident using Microsoft Office Suite.
  • Technically savvy and able to utilise software programs on hand-held devices.
  • Desirable: Experience in Home Care case management or care partnering.
